Through sustainable, community-based care packages, we ensure that people can continue to live in their own communities, benefiting from highly personalised healthcare that respects their unique needs.
Clients
Working closely with our NHS partners and families, we take a personal approach to nursing and healthcare support. Our model is built around collaboration and transparency, and we are proud to provide a service that considers the specific needs of each person we support. We are proud to support all local NHS Wales Health boards as well as Fostering agencies and private Case Management companies.
From the very first moment we receive information about a client’s needs, we work closely with the health board and, whenever possible, participate in MDT meetings for discharge planning. By doing this, we can anticipate each client’s unique needs and get to know them as individuals. This approach helps us match our staff’s competencies and personalities with each patient’s requirements, encouraging long-term relationships that enhance trust and understanding.

We recognise how important supporting our staff to be their best is and we are dedicated to keeping our team happy - as well as providing CPD accredited training opportunities, ongoing mandatory training courses and access to a variety of different patients with differing needs, our candidate care team is committed to ensuring our staff feel supported. Our staff have the flexibility to work part time or full time, ad hoc or consistently within our care packages across South Wales.
